What is endometriosis?

Endometriosis is a chronic, painful, and incurable disease that affects women worldwide, and it can happen anytime from their first period (menarche) to the last (menopause). It can also affect transgender men and non-binary people who menstruate.

It is the formation of endometrial-like tissue, found in the inner lining of the uterus, and causes pain in the pelvic area, inflammation, scarring, and, in the long run, it can also cause infertility.

It is currently not curable, but its symptoms can be treated through medication and proper care. Its symptoms can also be treated using Ayurveda by balancing doshas, detoxification, and can also help with the symptoms causing infertility. But first, let’s understand the reasons for endometriosis.

Causes of endometriosis

The cause of endometriosis is not known, but research shows that weak immune functions. People with endometriosis have a family history of endometriosis and other immune-mediated diseases like inflammation in the bowel and multiple sclerosis, etc.

According to Ayurveda, endometriosis is caused by an imbalance in the Apana vata, which is a type of Vata dosha. Apana vata is responsible for the flow of bodily and menstrual waste downward, and an imbalance of which causes the accumulation of toxins and causes tissue to grow in places it is not supposed to grow.

Symptoms and diagnosis

The symptoms of endometriosis are very large in number, and it is not easily diagnosed, even by the health care professionals. In many people, endometriosis remains asymptomatic, and others may not be aware of the disease itself despite having symptoms. The main symptoms are

  • Painful cramps during menstruation
  • Heavy bleeding during the menstrual cycle
  • Pain while peeing or pooping
  • Pain during sex
  • Infertility
  • Abdominal bloating

To diagnose endometriosis, it can take 4 to 12years because of the lack of technology for early diagnosis and treatment. Other methods, like ultrasound and MRI, are used to determine the size and the location of the tissue formation.

In case of knowledge of the exact location of the tissue, surgical methods can also be used to remove it, but in most of the cases, medication is recommended before any kind of surgery. In modern medicine, to diagnose the disease early, methods like blood tests and self-testing methods like saliva and menstrual blood tests are useful.

In Ayurveda, holistic approaches like Prakriti parikashan, Vikruti parikshan, Nadi parikshan, and clinical symptoms are taken into consideration. For treatment balancing the doshas and treatment of the root cause, the apana vata is the main part.

Treatment

It is mentioned that there is no known cure for endometriosis, so its symptoms are treated to reduce pain, scarring, and increase the chances of infertility. The treatment of endometriosis depends mostly on the age, severity of the symptoms, pain, and future plans of the person for pregnancy.

The following medications are used to treat the symptoms of endometriosis-

Non-steroidal anti-inflammatory drugs- These NSAIDs help reduce the pain only, and the other methods are the use of different hormonal therapies, which reduce the pain and also suppress the menstrual cycle.

Hormonal therapies include-

  • Hormonal contraceptive- This method includes estrogen and progestin therapy or only progestin therapy. It can be given in different forms like oral pills, IUDs, patches, implants, vaginal rings, birth control shots, etc.
  • GnRH analogues- These analogues first stimulate and then desensitise the pituitary gland to reduce the production of sex hormones that, in turn, suppress the menstrual cycle. Basically It stops your reproductive cycle to help you with the pain. An oral GnRH analogue is Orilissa, and the other analogue, which is injected into your skin, is Lupron.
  • Danazol- This is another kind of hormonal medication that stops the hormones that cause periods. After taking this medication, you may have a period once in a while, or it may fully stop.

Endometriosis Treatment in Ayurveda

Ayurveda mainly focuses on the treatment of the root cause of any disease, and it is called NidanaParivarjanam. Ayurveda is said to totally cure endometriosis. It is focused on balancing the doshas that cause the disease and following a healthy lifestyle. The following methods are used to treat endometriosis treatment in Ayurveda -

  • Vata anuloman- In this Panchakarma, like vasti chikitsa, uttar vasti, and medicines are used to channel Vata.
  • Treatment of vataj yoni vyapad, with appropriate medicines.
  • Sopphar chikitsa is used for the treatment of inflammation.
  • Rasayan chikitsa is used to stop the disease from coming back.
  • Raktaprasadan is used to cleanse toxins from the blood.

Along with these treatments and medications, other treatments are also used according to the constitution (Vata, Pitta, Kapha) of the individual.

Surgical treatment- Surgery for endometriosis can be an effective way to reduce pain and increase the chances of pregnancy, but there are always risks associated with surgery. Different types of surgery are recommended by the doctors.

  • Laparoscopic surgery- In this type of surgery, surgeons make a tiny cut in your abdominal skin and insert a thin tube called the laparoscope. This helps in finding the exact location of the endometrial tissue with a high-definition camera, so that the surgeon can insert other surgical instruments and remove the endometriosis.
  • Hysterectomy- Hysterectomy is the process of removal of the ovaries and uterus in severe cases of endometriosis.

Endometriosis is a chronic disease, and even after surgery, it can come back after a few years or even early. It depends on how bad it was. After it starts to grow, your health care professional can recommend a repeat surgery along with some medications. After surgery, many people get a big relief from pain.
